Event Planning Form
Use this form to alert the office of your upcoming event. Turn this form in at least 30 days (earlier the better) before your event!

Membership Form
Our church has a formal membership that we invite you to consider joining.

Reumbursement Form
If you have spent money on something for the church and you need to be reimbursed, please fill out this form and turn it into the church office.

Facility Use Form
For groups who wish to use our facility for an event, please fill out this form and return it to the church office.

Hold Harmless Form
This form is only necessary if requested when using the church facility for a large non church sponsored activity.

Wedding Policy and Application
This policy contains the necessary information for all those who desire to hold their marriage ceremony at the church or be married by a church pastor.

Child Protection Policy
This is the child protection policy of Grace Community Baptist Church. All persons working with children 0-18 years must review and turn in a signature sheet.

Child Protection Background Check form
Use this form to have the church conduct a background check to approve you for working with children.
